That's teh Docter UWA 12.5mm

Docter a brand name of Analytik Jena of "Carl Zeiss Jena" fame after the reunification agreements about the use of the Zeiss name.

This (or the previous version at least) was designed to go with their ASPECTEM 80/500 Spyglass (a sort of binocular spotting scope) e.g.
