Leica Televid 77 Ever-ready case (1 Viewer)

I just picked up a used APO Televid 77/zoom and ever-ready case. Everything is fine except my case seems to be a bit odd There is a little Velcro square patch on the top of the case between the snap for the focus cover and the velcro fro the lens front. What is supposed to fit here?

My case also lacks a carrying handle. I think I can jerry-rig something using some nylon webbing, but could anyone share a picture of what the handle is supposed to look like?
