Amazon.com said that this was for the H1, however I heard it was also for the DSC-H5 (which is the camera I have). Can anyone confirm/deny?

Also, do you need an adaptor for it to fit on the camera or does it just screw right on?

My understanding is that the camera should come with a plastic lens adaptor; see the "In the box" section here :

This adaptor should allow the teleconverter to screw in (58mm thread).
A more robust aluminium version of the adaptor is provided by pemaraal if worn or damaged plastic threads are a concern :
